- Bloosky Interactive, LLC
The Circuit Court of Cook County, Illinois (the “Court”) authorized this Notice. This is not a solicitation from a lawyer or a lawsuit against you.
YOUR LEGAL RIGHTS AND OPTIONS IN THIS SETTLEMENT | SUBMIT A CLAIM FORM | Submitting a “Claim Form” is the only way to get a payment. If you elect to receive a payment from this Settlement, you will give up your right to be part of any other lawsuits involving these claims. | EXCLUDE YOURSELF | You can ask to be excluded from this Settlement. That means that if the Settlement is approved, you will not receive a payment. However, you will keep the right to file your own separate lawsuit involving the issues in this case if ever you should wish to do so. | OBJECT | You can object by writing to the Court about any reasons that you have against the Settlement. | ATTEND A HEARING | You can attend a hearing and request to be allowed to speak in Court about the fairness of the Settlement. | DO NOTHING | If you choose to do nothing, you will NOT receive a payment AND you will give up your rights to file a lawsuit regarding these matters in the future. |
