The Circuit Court of Cook County, Illinois (the “Court”) authorized this Notice. This is not a solicitation from a lawyer or a lawsuit against you.

YOUR LEGAL RIGHTS AND OPTIONS IN THIS SETTLEMENT
SUBMIT A CLAIM FORM	Submitting a “Claim Form” is the only way to get a payment. If you elect to receive a payment from this Settlement, you will give up your right to be part of any other lawsuits involving these claims.
EXCLUDE YOURSELF	You can ask to be excluded from this Settlement. That means that if the Settlement is approved, you will not receive a payment. However, you will keep the right to file your own separate lawsuit involving the issues in this case if ever you should wish to do so.
OBJECT	You can object by writing to the Court about any reasons that you have against the Settlement.
ATTEND A HEARING	You can attend a hearing and request to be allowed to speak in Court about the fairness of the Settlement.
DO NOTHING	If you choose to do nothing, you will NOT receive a payment AND you will give up your rights to file a lawsuit regarding these matters in the future.

These rights and options—and the deadlines to exercise them—are explained in this notice.
The Court still has to decide whether to give final approval to the Settlement. If it does, and after any appeals are resolved, the benefits discussed in this notice will be given to those who have not excluded themselves from the class and who have submitted valid claims. Please be patient. It may be some time before the Settlement becomes final.
